About this project…
This website is a product of the Enhance the Coordination of Care Action Team, one of the 12 Action Teams sponsored by the Northwest Community Health Innovation Region’s (NWCHIR) Behavioral Health Initiative (BHI). This Action Team was launched in November 2021 at the Behavioral Health Initiative Action Summit. It was inspired by the results of the MITHRIVE community survey and focus groups with community health workers and other behavioral health professionals which revealed that, for consumers and workers alike, the health and human services network is difficult to navigate. The goal of this project is…
to enhance the collaboration and coordination between care coordination groups and agencies in the region. An interconnected network is a strong network. Given the high turnover rate for care coordinators in the region, groups like the ones listed on this website can help preserve institutional knowledge. These groups are also vital resources for experienced community health workers since the available resources and their requirements change regularly.
